How do I request an academic accommodation?

Answer

1. Read the Accommodation Process Instructions

2. Complete Registration Form if you are a new student registering for the first time, or complete a renewal form Accommodation Memo Renewal Form (fillable PDF) if you are renewing your accommodations from last semester. 

3. Obtain Provider/Supporting Documentation, Please include the diagnosis and diagnosis code (You have the option of completing the Documentation of Disability Form or you can request a letter from your physician). If you are renewing, this is only required if you are requesting changes to your memo.

4. Obtain a copy of current class schedule through Elucian

5. Submit all documentation through the Student Accommodation Portal*

*If you do not have a USD student email, send the completed forms to accessibility@usd.edu. You must put your full name in the subject field.
