Environment

• Sodexo
• Transact
• Vendors
• Contractors
• Department of Health employees (DOH)
• Vermillion Police Officer
• Wellness Center
• South Dakota Public Broadcasting (SDPB)

Resolution

1. Department will Contact HR with
   • Full Legal Name (include maiden name if applicable)
   • Date of Birth
2. HR will reply with each person's Banner ID number 
3. Payment
   1. If the department pays for the card, please provide the index number to the Vendor or Affiliate
   2. If the department does not pay, the Vendor or Affiliate should bring payment with them
4. Employee can now go to the card office with their Banner ID number and payment
   • I D Weeks Library Room 104
   • Monday - Friday 8 AM - 5PM
