Preferred Name Change

Issue/Question

My account shows up with my parent's first name

How do I change my preferred name 

Environment

• USD account
• Email
• Academic Records

Cause

Would like my preferred name to be reflected in USD systems

Resolution

Note: Your email address, username and display name can be changed without the loss of account access, emails, and other data 

1. Log into Self Service
   • Students 
   • Employees
   • Faculty
2. Click in the upper left corner 
3. Click Banner
4. Click Personal Information
5. Choose Preferred Name 
6. Fill in the Preferred First, Middle and/or Last Name
7. Click Verify
8. Your preferred name change requests will go to HR and Registrar respectively and get reviewed/approved
9. Once approved your display name will update automatically, if you would like to update your username and email address, please Click the Email Address Name Change button on this page  to update your email address
   1. Provide the following information
      • Preferred first name
      • legal first name
      • Legal last name 
      • ID number
      • When (date and time) you would like the change to process
