Requesting Emotional Support Animal (ESA) Approval

Issue/Question

How do I request ESA approval for on-campus housing?

Cause

Students may be allowed an ESA in on-campus housing upon approval from the Office of Accessibility and University Housing. Students will need approval from the Office of Accessibility before requesting approval from University Housing.

Resolution

  1. If you are registered with the Office of Accessibility, then log in to Accommodate
  2. If you have not registered with the Office of Accessibility, then you will need to do so before requesting ESA approval

  3. Click Resources from the menu on the left side of the screen
  4. Select Resource Library from the drop-down
  5. Click ESA Process in the center of the screen and read the downloaded document
  6. Click ESA Form Step 1 and 2. A new tab will open with the disability acknowledgement form. Read and complete the form
  7. Click ESA Step 3. A new tab will open with a form for your treatment provider. Send the form to your treatment provider for completion
  8. Click Accommodation from the menu on the left side of the screen
  9. Select Supplemental
  10. Click Add New in the center of the screen
  11. Answer the populated questions
  12. Click Add Item to upload completed documents
  13. Click Submit

Note: The Office of Accessibility will process your submitted request as soon as possible. After reviewing your submission, the Office of Accessibility will reach out to you via your student email in order to arrange a meeting (in-person or virtual). The meeting's purpose includes the opportunity to learn more about your experience, to discuss potential accommodations, and to engage in the interactive process.
