How do I request a housing accommodation?

Answer

  1. Read the Accommodation Process Instructions

  2. Complete Registration Form

  3. Download and complete the University Housing Accommodation Request Verification form Parts 1 and 2 (Download the PDF, do NOT open in Google docs)

  4. Send University Housing Accommodation Request Verification form Part 3 to your provider (Download the PDF, do NOT open in Google docs)

  5. Return all documentation in one submission through the Student Accommodation Portal*

*If you do not have a USD student email, send the completed forms in one email to accessibility@usd.edu. You must put your full name in the subject field.
