I am wanting to change a template and remove a user.  

Please use the attached form to update DocuSign 

Please update the vendor template in DocuSign 

Template is missing

Please contact the Service Desk to request changes to DocuSign templates

  1. List the template to be changed
  2. Indicate all changes needed
  3. Indicate whether your department is the functional owner of the template
    Note: Check to see if the template is in Shared With Me folder and only missing from a custom folder
