1. Form Creation for USD Website
   1. Email web@usd.edu with the following information to make this request 
      • Information on what the form is for
      • What fields are needed on the form
      • Which fields are required
      • Who (university email address) the form should be assigned
      • If available attach the current form
2. Requesting a new form within Coyote One Stop
   1.  Email Service Desk with the following information: 
      • Information on what the form is for
      • What fields are needed on the form
      • Which fields are required
      • Who the form should be assigned
      • If available attach the current form
