Early Withdrawal (International Students)

Overview

International students, please use the Early Withdrawal form if you would like to take a leave of absence or otherwise withdraw from your program of study. Failure to submit this form before you withdraw may result in your SEVIS record being terminated for unauthorized early withdrawal.

By filling out this form, you certify the information given is accurate and understand the following:

  1. It is your responsibility to complete all necessary steps to withdraw from your classes through the Registrar’s office, withdraw from on-campus housing or notify your landlord, and settle your bill (if applicable) at the Business Office;
  2. Your SEVIS record shall be terminated based on the date of withdrawal that you have indicated in the form;
  3. You must depart the United States no later than 15 days after the withdrawal date;
  4. You cannot work on or off campus once you withdraw from your classes or program; and
  5. In order to return to the U.S. and resume study at the University of South Dakota, you may need to apply for readmission, obtain a new SEVIS I-20, and repay the SEVIS fee.
