USD is dedicated to ensuring that all students have equal access to educational opportunities and campus resources. This service provides a range of accommodations and support to individuals with disabilities, fostering an inclusive and supportive learning environment.
Students must self-disclose their disability and provide professional documentation to receive accommodations.

Choose from the list of services on this page and complete the relevant forms for the accommodation(s) requested.

Read the accommodations process instructions carefully before submitting your request. You may also want to review important information and FAQ on the Office of Accessibility webpage. You can self-book a meeting with the USD accommodation coordinator at any time in the process.

New students need to complete the initial Office of Accessibility Registration Form (fillable PDF). Note: IEPs and 504 plans are specific to high school. They do not transfer to USD. Accommodations will be provided based on your disability and the medical documentation you provide to ensure you have access to all USD programs and services.
Returning students with no changes in their accommodation determination need to download and complete the Requesting Accommodation Memo Renewal (Fillable PDF).

If you would like your information released to a third party (parents, health care providers, etc.), download and complete the Release of Information Form and upload along with other documents in step 5.

Upload all forms at one time including health care provider documentation using the secure Student Accommodation Submission Form. You will need to log in with your USD email address to access the portal page.
Note: If you do not have a USD email address yet, the documentation can be obtained from your healthcare provider, licensed counselor, or therapist and emailed to our office at accessibility@usd.edu.
