PL/SQL Invalid Username/Password Error Message

I am unable to login to SQL

Error: ORA-01017: invalid username/password; logon denied

Error: ORA-28000: the account is locked

1. Contact the Service Desk with the following information
   1. Environment connecting to (Banner Production, Banner Test, etc.)
   2. Any Error messages you may be receiving
