An index number would not be needed as Sodexo covers the cost of Bonus Flex.  It can be noted that the bonus flex is for the Sodexo survey winners.

1.  Please contact the Service Desk with the following information: 
   1. Winners Name
   2. ID Number 
   3. Amount won
