Consent to an electronic W-2 and electronic 1095-C form

Resolution

1. Log into SNAP with your USD email address and password
2. Click Employee 
3. Below Employment Details click  Employee Self-Service
4. Click Tax Forms
5. Click Electronic W2 Consent
6. Select what you would like to receive electronically
   • Consent to receive W-2 electronically: 
   • Consent to receive 1095-C electronically: 

Note: 1095-C is the form needed for income tax return to prove insurance coverage
