Resolution

  1.  Please contact the Service Desk with the following information: 
    1. Name of user Qualtrics Account
    2. Name of Survey to be restored
    3. Are the Surveys Deleted Shared with User
      1. Owner of Survey
  2. Once IT restores survey owner of survey will need to share the survey to appropriate people again
