  1.  Please contact the Service Desk with the following information: 
    1. Employee name
    2. Employee email
    3. Employee phone number
    4. DocuSign groups to be assigned (Admissions, CDE, Human Resources, Housing, etc)
    5. DocuSign templates that will be accessed by the user - if known
