  1.  Please contact the Service Desk with the following information: 
    1. Gather as much information as possible from the Service Provider support
      1. Name of the service
      2. Purpose of the service
      3. If they are a member of InCommon provide their EntityID
      4. Provide any metadata
      5. Which attributes need to be released
      6. Any detailed instructions
      7. Provide support contact information
