I am receiving an error when trying to upload my thank you letter 

Error: Account Creation Failure. An error occurred during your account creation process.

  1.  Please contact the Service Desk with the following information: 
    1. The steps you are taking to access the page
    2. Error message you are receiving 
    3. Service Desk will contact the Scholarship Manager administrator who can address this issue
