Requesting Fields added to Salesforce

Issue/Question

The end user is requesting new fields to be added to Salesforce because of a change in processing.

A new field needs to be added to the Salesforce interface.

   1. What object will the fields be associated with
   2. What are the fields
   3. What type of fields are they, i.e. date, checkbox, text, picklist
   4. If the field is a picklist, what are the values for the picklist
   5. What page layout and section will the fields be associated with
   6. Who should have access to the new fields
   7. What system the new data is coming from and how to find the data
   8. Logic needed to calculate a value
